Nagpur subscribers have not responded to TRAI’s CAF Threat

Not even a single CAF form has been submitted to government in Nagpur, a Phase –II city in Maharashtra. If your consumer application form (CAF) is not submitted to revenue department before July 10, TRAI will direct the multi-system operator (MSO) like UCN, BCN, GTPL, etc. to disable your set top box (STB). If the MSO doesn't disable your STB action will be taken against it.
However, local cable operators (LCOs) have squarely blamed consumers and MSOs for the delay and are accusing the government of pressuring them. They have warned of a strike in coming days. LCOs in Kolkata and Delhi had gone on strike on Sunday. Additional collector Asha Pathan told TOI on Monday evening that the revenue department had not received a single CAF. "The local cable operators (LCOs) are supposed to collect CAFs from subscribers and deposit it with MSOs, who in turn should provide a soft copy to us."

The cable operators stand to lose if CAFs are submitted. You have to mention your choice of channels. When this information is compiled, channels will come to know of their actual viewership, which in turn will help them fix the pay TV charges to be paid by MSOs. 
LCOs have, however, blamed consumers for delay. They say that Cable channels are repeatedly flashing that consumers will have to submit two photographs and photocopy of address proof along with the form. However, many of them don't have these things when Cable Network workers go to their homes. This is delaying the process. It seems that they will cooperate with operators only when their STB is disabled. 
Operators have also blamed MSOs who are yet to finalize channel packages. They say that subscribers do not know what and how to select the channels as information is not available at all.
